We are committed to providing you with the finest care possible. To achieve this, we need your assistance and understanding of our payment policy.

Our well-informed staff is happy to help you complete your insurance forms and will work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. We require an estimated deposit when treatment is completed, and any excess payment will be promptly refunded to you.  Please call our office with any questions.

If your insurance company does not participate, please remember that you are responsible for payment at the time of treatment. Insurance is a contract between you, your employer and the insurance company. We are not a party to that contract. You are responsible for payment regardless of any insurance company’s arbitrary determination of Usual and Customary Rates.  Please remember to bring your insurance information so that we can expedite reimbursement to you.

For your convenience, we accept Visa, MasterCard, American Express, Discover, check or cash payment. The fee must be paid in full upon completion of treatment.
